Contracting Forms

Behavior contracts can be written in a variety of ways, but we recommend using a standard format to ensure that all key components are included. Three variations of the contract form used in Let’s Make a Contract can be downloaded here for free.

Tasks & rewards Forms

Free forms for identifying tasks and rewards for contracts can also be downloaded here.

Families can choose contract tasks and rewards with an informal discussion or use a more structured approach that involves three forms, My Tasks, Your Tasks, and My Rewards.

My Tasks Form

Each family member writes things they do now to help the family on the left side and writes things they could do in the future to help on the right side.

your tasks

Your Tasks Form

Each family member fills out the Your Tasks form for everyone else in the family, but not for themselves. Everyone should write at least one thing on each side of everyone else’s Your Tasks form.

Each family member lists things they would enjoy as a reward for completing the task. The list can be divided into categories such as activities, outings, and special treats.
